I am the parent or guardian of the member being registered. I agree to leave the coaching of soccer to the coaches. I understand that the person being registered is playing soccer at his/her own risk and I hereby indemnify the DSCJ and its members should any injury befall him/her while under the control of the DSCJ.

I acknowledge and fully understand that unless I expressly notify the DSCJ in writing, I will be deemed to have granted my express consent to allow the DSCJ to take photographs of individual players and groups of players and/or teams from time to time, as part the material used to promote the DSCJ through various forms of media and as part of its website, whilst taking all reasonable measures to protect the identity of the players.

I hereby declare that I have answered the above questions truthfully and I am the legal guardian and duly authorized to sign up this player. I agree to be bound by the constitution and the rules and regulations, especially the Code of Conduct of the DSCJ and I undertake to give written notice should the member resign and pay any money due by the member to the DSCJ. Therefore, clearance certificates will only be granted if all outstanding fees have been paid in full.
